Eligibility and Referrals to the Community Options Program

Eligibility:  

• The program is designed to meet needs of the elderly, adults with physical disabilities, and persons with developmental disabilities.  Persons under 65 must have a disability determination from Social Security. 
• The individual must have long-term health needs which would require a nursing home level of care if they were not to receive support at home. 
• A person must also be financially eligible.  There are income limits, which change yearly. The asset level does not include a person’s home, car, or some other exempt assets. 

Referrals:  A referral to COP may be made by an individual, their family member, friend, doctor, hospital social worker etc.  If a person feels that they or someone they know would be eligible for COP, they can make a referral by calling Kewaunee County Human Services COP Unit at 920-487-7252.
