Procedures After a Referral Is Made to COP 

  1. Once a referral is made to the county COP unit, a long term support care manager will set up a meeting for an assessment.  At that the assessment a care manager will meet with the individual who was referred and family members if desired. 
  2. The care manager will complete a functional screen to determine if the person’s health needs meet the requirements. 
  3. A health form will also need to be completed by a trained medical professional. 
  4. If a person meets the functional screen requirements, a meeting will be set up with a county economic support worker to make sure that the person is financially eligible for the program. 

If a person has passed all the requirements and been accepted on the Community Options Program, services and supports will be set up
