Services for Recipients of the Community Options Program

 Services are tailored to meet the needs of each recipient; services must be cost effective, and are also dependent on availability of funds.  The following list are examples of possible services.

• Medical assistance to cover medical costs not covered by Medicare or other insurance. 
  • Medical care, home health care, certain medical supplies, medical transportation, and therapy are examples of items for which medical assistance could pay.  I
• Case management
  • The care manager will have monthly contact with the recipient to provide continued assessment of needs and make sure all necessary services are in place.    
• Supportive home care
  • Supportive home care involves having a worker come into the home to help with meals, cleaning, laundry, or errands.  Clients who need help with personal cares such as dressing, grooming, etc will receive this assistance. 
• Adaptive equipment
  • Examples of equipment that the program can help provide are:  walkers, wheel chairs, or commodes, and Lifeline or other personal emergency response systems.
