By execution of this permit application, applicant acknowledges having reviewed, fully understands, and agrees to comply with the terms and conditions set forth on this permit application.
Applicant is aware of, and shall fully comply with, all applicable laws, codes, regulations, ordinances, rules and order.
Applicant shall obtain the insurance coverage(s) considered sufficient by the Highway Commissioner with suitable insurance companies and shall provide a certificate of insurance as evidence of such coverage(s) to the Highway Commissioner.
Applicant shall pay for restoration, to a condition satisfactory to the Highway Commissioner, of any pavement, bridge, culvert, sewer pipe or other improvement damaged by reason of the use of the highways by the applicant.
Applicant agrees to indemnify and hold Kewaunee, its employees, officers, and officials, harmless from and against any and all claims, costs, expenses, actions, causes, liens, liabilities, damages, judgments, and attorneys’ fees arising from or connected with property damage, personal injury or death arising from the granting of this permit or that may arise from or on account of any act done or omission pursuant thereto.
The Highway Commissioner may suspend or revoke this permit for good cause.
