A Community Health Assessment (CHA) is a process in which data about the health of Kewaunee County is collected and analyzed, resulting in a report that helps guide decisions on how to improve the community’s health.

Community Health Improvement Plan (CHIP)

A Community Health Improvement Plan (CHIP) is a local plan that outlines strategies to address the priority health needs identified in the CHA, incorporating community input and available data to guide efforts that improve the overall health of Kewaunee County.
