Kewaunee County Launches Health Needs Survey

KEWAUNEE, WI – The Kewaunee County Public Health Department and the University of Wisconsin-Madison Extension have teamed up to launch a health needs survey and they are asking the public for their participation.

The short six question survey will be used to evaluate public perceptions of the County’s top health needs and develop the next 5-year Community Health Improvement Plan (CHIP), which entails identifying and analyzing community health needs, prioritizing those needs, and then implementing a plan to address unmet needs.

The survey asks participants three health questions along with demographics, and if they have interest in participating in a Community Health Assessment on December 16, 2021 hosted by the Public Health Department, which will help guide the path that Kewaunee County takes toward improving the health of the community.

“This is an opportunity for community members to help decide what health priority areas are of greatest concern. When we gather members of the community together, it creates partnerships. Through the data presented and ideas shared, we can develop a plan that best suits the needs of Kewaunee County,” said Cindy Kinnard, RN, Kewaunee County Public Health Department Director.

Participation in this survey is voluntary and confidential. Individuals will not be identified on any prepared reports. Participants must be 18 or older.

The survey closes on Friday, November 5, 2021.
